I looked at several PHI accounts both at 505 14th Street and 180 GRAND AV, besides the DSL account 510-663-**** disconnected on 07-15-05. None of the accounts has any notes indicating that either PHI or its agent ATN ever requested to disconnect BTN 510-663-**** until you called a few days ago. Nor do they have any notes referencing any request from ATN to move any PHI accounts. On BTN 510-663-**** itself there is a note dated 07-12-04 saying that agent ATN Pete called to review account. This is insufficient to justify an adjustment. You may want to contact ATN to see if they ever placed the disconnect order. If you find a proof of your request to disconnect 510-663-**** in August 2004, please contact our billing department at 800-505-5400.
